How to Use — Quick Guide

1. Fill Company Information — Name appears on the report; add site/location and hours for KPI context.
2. General Information — Set date/time, department, reporter, and any witnesses.
3. Incident Details — Select type & severity, write a clear description, and choose all relevant Contributing Factors (multi-select).
4. Immediate Actions — Document what you did immediately to make things safe.
5. Root Cause — Choose a method (5 Whys, Ishikawa, etc.) and summarize your verified root causes.
6. CAPA — Add one or more actions with owner, due date, status, and priority. Use “Add CAPA”.
7. Attachments — Upload supporting photos/files (stored in the page context; Blogger-safe).
8. Review/Approval — Record investigator and approver details and dates.
9. Add Incident — Click Add Incident to save. It appears in the log and report automatically.
10. Generate Report — Open the “Generate Report” tab and click Print / Save PDF.

Tip: On desktop, hold Ctrl/Cmd to select multiple contributing factors. On mobile, tap options one by one.
