Everyone calls me Tero. I am a Health Safety Environment Practitioner with over 6 (Six) years of practical experience across the maritime oil and Gas and Construction Sectors. The OSHA Fails blog is not by Accident. The rationale behind OSHA Fails is to help people learn from the many mistakes that lead to incidents especially at the workplace. Indeed it is not the ideal for people to learn from accidents, someone may suggest.

But accidents can become lessons that serve both reactive and proactive needs. Learning from accidents serves a good purpose because it increases awareness and people are able to appreciate what happened to others so they do not repeat same. The name OSHA Fails is to reiterate the fact that a failure to comply with Occupational Health and Safety Regulations will promote a poor safety culture leading to incidents. Additionally, I have created OSHA Fails  to make available  free tools , video lessons of fails , documents and articles/blogposts. 


My HSE Journey

I am happy to say that I have been practicing HSE for a period of 8 Years and still counting. I am currently a Certified Member of the Institution of Occupational Safety and Health and a Founder of GLOBAL HSEQ PROFESSIONALS, a social media–based professional group with professionals from UK, US, Trinidad and Tobago, South Africa, Kenya, Ghana just to mention a few.
