I am a Health Safety Environment Practitioner with over 6 (Six) years of practical experience across the maritime oil and Gas and Construction Sectors. The OSHA Fails blog is not by Accident. The rationale behind OSHA Fails is to help people learn from the many mistakes that lead to incidents especially at work and home. Indeed it is not the ideal for people to learn from accidents.

But Accidents can become lessons that serve both reactive and proactive health and safety needs. Learning from accidents serves a good purpose because it increases awareness and people are able to appreciate what happened to others so they do not repeat same. I came up with the Name OSHA Fails to reiterate the fact that a failure to comply with Occupational Health and Safety Regulations will always create a poor safety culture leading to incidents.

Additionally I have created OSHA Fails to share Free Resources for Occupational Health and Safety and Environment Professionals to Utilize in creating Safer Workplaces and a Safer Home too. I will share Documents of all formats to champion this cause. On OSHA Fails you will have video content, text content in the form of Plans, procedures, HSE files, training materials, excel dashboards and a blog where I can share my personal experiences and react to incidents that occur in other workplaces. Finally i will publish HSEQ Jobs.

My Work Experience

I Have worked as:

  • HSE Superintendent/DDPA in Charge of 1 Offshore Supply Vessel, 1 Bulk Carrier (Landing Craft) and 2 Tugboats.
  • Onshore HSE Advisor for an Oil and Gas Welding and Fabrication Company.

I am happy to say that I have been practicing HSE for a period of 8 Years and Still Counting. I am currently a Certified Member of the Institution of Occupational Safety and Health and a Founder of GLOBAL HSEQ, a Social Media based Professional Group with Professionals from UK, US, Trinidad and Tobago, South Africa, Kenya, Ghana just to mention a few.
